How do you tell which customers are VAT-liable?

Hello all
For (UK-based) creators, how do they tell how many customers are VAT-liable so that the creator can collect VAT accordingly? Are they even meant to collect VAT if the item is imported from overseas? The Customs would then ask the backer to pay VAT - right?
