Urgent!!! PLEASE answer my Indiegogo technical question.

PLEASE I NEED HELP! My indiegogo campaign is launching on Tuesday and I am having a big technical issue. Please someone help me.

Here is my problem:
I have 14 different styles of bags and 500 "early birds" bags in stock. I cannot create 14 different perks for each style. So what I want to do is create 1 perk for all the 500 bags. When people pledge, I want to email them a form to fill out where they can say how many bags they want and the quantity. Then I want a code to embedded on my project page so that a form with the stock still left is showing and people accordingly.
So in brief:
1) I want to create a form where people can choose the style(s) and quantity they want.
2) I want that form to be linked to my project page so that the stock can be updated automatically as people order.
3) Not sure how people will pay but should the form have the ability to process payment too?
Can someone PLEASE tell me how to do that or help me do it?
